Service Delivery Location contact

Derived from RMIM: _ and HMD: _

ServiceDeliveryLocation
Design Comments: An identification of a service location (or facility) where health service has been or can be delivered. E.g. Pharmacy, Medical Clinic, Hospital
UsageNotes: Information other than the id is expected to be extracted from the service delivery location registry.
Rationale: Used for tracking service delivery responsibility, to provide contact information for follow-up and for statistical analysis. Also important for indicating where paper records can be located.
classCode [1..1] (M)
Role (CS) {CNE:C:RoleClass:SDLOC, fixed value= "SDLOC"}
id [1..1] (M)
Role (II.PUBLIC)
Design Comments: Unique identifier for a healthcare service location.
Rationale: Allows for lookup and retrieval of detailed information about a specific service location. Also ensures unique identification of service location and is therefore mandatory.The identifier is mandatory because it is the principal mechanism for uniquely identifying the facility.

code [1..1] (M)
Role (CV) {CNE:V:ServiceDeliveryLocationRoleType}
Design Comments: Describes the basic type or category of the service delivery location.
Rationale: Useful in providing context. May also be used for grouping and organizing data. Because this is a key element in understanding the location, it is mandatory.
addr [1..1]
Role (AD.BASIC)
Design Comments: The information by which a service location may be contacted either physically or by mail.
Rationale: Identifies the physical location of a service location and also allows for the location to be contacted.The address is marked as 'populated' because it is considered a critical piece of information about the facility, but may not always be available or meaningful.
telecom [1..5]
Role (SET<TEL.PHONEMAIL>)
Design Comments: The phone numbers and/or electronic mail addresses by which a service location may be contacted.
Rationale: Allows a service location to be communicated with and is therefore important. Because a contact number won't always exist, the field is marked 'populated'.
location [1..1] (Place)
subjectOf [0..5] (Subject)
Design Comments: Identifies the position of the service delivery location according to a set of geographic coordinates.
Rationale: Allows capturing of GPS, legal land, and/or range-meridian coordinates of a location for subsequent proximity searching. Also allows the capture of disaster sites and other non-dedicated service delivery locations which may not have a traditional address. (These locations are typically registered for public health purposes.)The association is optional because not all systems will choose to support capturing information at this level.

name [1..1] (M)
Place (ST)
Design Comments: The name assigned to the service location.
Rationale: Used for human communication, and for cross-checking of location Id and is therefore mandatory
